To see one of our clinical staff you need to make an appointment.
When you ring the clinic on our main telephone number 09- 5219811, the recorded
message will direct you to the appropriate receptionist or secretary according

On the day of your appointment please bring your referral letter, ACC form, and any xrays or MRIs relating to your injury. If you must cancel an appointment, it would be appreciated if you would notify our reception staff as soon as possible, preferrably the day before.

Accident Compensation
Commission - ACC
An accident is defined as a sudden event, in which the application
of an external force causes an injury, that has occurred at a particular time and place. This may have been at home or at work, during your leisure time or while playing sport. At your first visit to a doctor or physiotherapist,you will be asked to fill out an ACC form and, once accepted by ACC, the resulting subsidy will help pay towards the medical treatment required for this injury.
Referrals
Often, you will have been to your own general practitioner or physiotherapist
initially for your injury and then have a referral (a letter stating your medical
condition) for one of our staff.
A referral is essential for the Sports
Physicians and the Orthopaedic specialists, but if you want an appointment with one of the Sports Medicine Registrars then
you can book directly with them.
For our Physiotherapy staff you may have a referral from a general practitioner,
however, it is also acceptable to ring for an appointment without having a referral.
For our other services of Footworks Podiatry and Massage Therapy, a referral is not required.

Parking
adidas Sports Medicine clinic is located within the grounds of the Colin Maiden
sports park in Merton Road, St Johns. See our
location section for directions to
the clinic. We are experiencing major ground works around our clinic through October to December 2009, we hope the disruption is minimal and recommend using Gates 6 or 3 for entrance to the grounds. There is plenty of off-street free parking available outside the clinic.
